Causes of Toxic Epidermal Necrolysis

WHAT CAUSES TEN? Toxic Epidermal Necrolysis

Levi, et a al., Pediatrics, "Medications at Risk of Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis in Children: A Pooled Analysis" 123 (2) e297-e304, e297 & e302 (2009)

Conclusions.  We confirmed 4 previously highly suspected drug risk factors for Stevens-Johnson syndrome / toxic epidermal necrolysis in children:  antiinfective sulfonamides, phenobarbital, carbamazepine, and lamotrigine.  Amoung more unexpected risk factors, we suspect that acetaminophen (paracetamol) use increases the risk of Stevens-Johnson syndrome or toxic epidermal necrolysis.  Pediatrics 2009; 123:e297-E304.

2. Foreman, et. al., Drug Safety, "Erythema Multiforme, Stevens-Johnson syndrome and Toxic Epidermal Necrolysis in Children," 25 (13) 965-972, 966 & 969 (2002) Reviewed 61 patients between 1985-1995. "All cases ofTEN were attributable to drugs." "All three of the disorders [EM, SJS & TEN] have been linked to drugs, with TEN being exclusively attributed to this factor."

3. Paquet, P, et al., "Treatment of Drug-induced toxic epidermal necrolysis (Lyell's syndrome) with intravenous human immunoglobulins," Burns, pp. 652-655 (2001) "The generally proven cause of TEN is an adverse drug reaction resulting from a specific formation of toxic drug metabolites, perhaps in the skin itself."

4. Garcia-Doval, et. al., Arch Dermatol, "Toxic Epidermal Necrolysis and Stevens Johnson Syndrome," Vol. 136, pp. 323-327, 324 (2000) "Both diseases [SJS & TEN] are primarily, if not solely, caused by drugs."

5. Roujeau, JC, Rev. Rhum. [Engl. Ed.], "Severe Drug-Induced Blistering Disorders," Vol. 64 (1); pp. 5-9, 7 (1997) "The herpes simplex virus is not a cause of Stevens Johnson syndrome, which is clearly related to a drug in over two thirds of cases."
